HUMULIN® R U-500 (insulin human): BD™ U-500 Insulin Syringe

A U-500 insulin syringe that does not require dose conversion is available from Becton, Dickinson and Company.

A BD™ U-500 Insulin Syringe is available from Becton, Dickinson and Company. Lilly also received approval for the updated United States Prescribing Information for Humulin® R U-500 (insulin human injection) 500 units/mL (U-500R) that includes branding changes to the U-500R vial and language changes for use of only the U-500 insulin syringe with the U-500R vial (Humulin R U-500 Instructions for Use, 2016). Pharmacies may not stock the U-500R vial or the U-500 insulin syringes, and thus 1 to 2 business days may be required for receipt.

Availability of BDTM U-500 Insulin Syringe and Humulin R U-500 Vial

BD™ U-500 Insulin Syringe

  • 0.5 mL syringe

  • U-500 symbol (green)

  • Green needle shield

  • Up to 250 units per injection

  • Numerical markings from 25 to 250 units

  • Dosing increments of 5 units per syringe dose line

  • 6 mm 31 gauge needles (no safety syringe)

  • Individual blister packs (NDC 08290-3267-30)

  • No other syringe can be substituted

  • Requires separate prescription (Figure 1) (Humulin R U-500 Instructions for Use, 2016; Humulin R U-500 Package Insert, 2016)

Humulin R U-500 Vial

  • 20 mL multiple dose vial (10,000 units per vial)

  • 1 vial per carton (NDC 0002-8501-01)

  • Band of aqua coloring on vial and vial packaging

  • Green “U-500” concentration statement

  • Green flip top

  • Red warning on the front panel

  • Statement advising use with only U-500 insulin syringes

  • U-500 insulin syringes must be prescribed for use with the vial to avoid errors that may arise from dose conversions (Humulin R U-500 Instructions for Use, 2016; Humulin R U-500 Package Insert, 2016)


The U-500 insulin syringe does not require dose conversion; the dose lines on the U-500 insulin syringe show the number of units of U-500R to be injected (Humulin R U-500 Package Insert, 2016).
